Yes entire replacement. They're much nicer than the stock ones.

The problem with the extension, at least the ones I tried on a loaner once, they're a little flimsy but more importantly add thickness so I found my fingers felt a bit cramped between the double paddles and the turn signal controls.

If you shop around you can find them for less.

They look pretty cool, if I go this route I'll def look into those for an option. Could see how the extensions could make the turn signal feel cramped

Ha - it's not a problem for the turn signal, reaching in front to get at the paddles I found to be annoying.
